Medical Records Request

To request a copy of your medical records, please download and complete the Medical Records Release Form using the button above. Once completed, attach the form to the contact form below and submit your request. Once received, we will process your request promptly.

Please note that California law allows up to 15 calendar days to provide copies of medical records. If you would prefer to view your records in person, we can make them available for inspection in our office within 5 working days of your request — just let us know and we will schedule a time with you.

A copying fee of $0.25 per page may apply. Thank you for your patience.
